Access for Individuals with Disabilities Online Services

Court users, including jurors, parties in a case, witnesses, lawyers and others, can receive reasonable accommodations from the Court if necessary to fully participate in Court programs or services. The Court normally requires at least 5 working days advance notification in order to provide an accommodation. If you are in need of an accommodation due to a disability please complete the form MC-410, Request for Accommodations by Persons With Disabilities and Response, and return the completed form to the Court’s ADA Coordinator by fax at (707) 263-2944 or by mail at 255 N. Forbes Street, Lakeport, CA, 95453. Or you may contact the Court by phone at (707) 263-2374, Ext. 2838.
